If you are wishing to Waive the Constellis / Triple Canopy Insurance Options, it has come to our, your Union Local 309 Officers – David Hemingway, David Tucker, and Jeffrey Meyer – that there is a quick form that needs to be submitted.
There is also confusion surrounding this process.
The completion of the form is simple and involves a check-mark, printed employee’s name, last four digits of your SSN, signature, and date.
This information, or the need for having this form filled out, came from Constellis’ benefits@constellis.com system.
The manner in which Constellis shared the insurance information has been confusing – from feedback both at the end of the automated Insurance Election Process and questions asked of Constellis personnel by phone.
I believe that if you want to waive the offered insurance that you should submit a completed copy of this form by C.O.B. (Close Of Business) Eastern Daylight Savings Time (two hours early or by 14.00 hrs. MDT) this Friday, 12-July-19.
I am aware that this is just one more piece of paper and it is frustrating. Consider taking a minute, filling it out, and emailing it to benefits@constellis.com
